You will also need to complete the application packet (add referenced forms). This packet will be due three weeks prior to the start of the school year.
The Michigan Department of Human Services (DHS) Central Registry Clearance form may take several weeks to receive back from DHS. Please complete and mail this form to your local DHS office as soon as possible. We expect all DHS forms to be received at school by the first week of the school year.
